I'd like to ask a question about a storyline I have planned for an RP that is on TVTropes. One of the heroic characters and one of the most nasty villains have had dealings in the past. The exact set of circumstances are below, though given there's a heavy fantasy element in the villains, I don't know how to handle this or how it looks.
The character is Andrew DeWitt, formerly Andrea DeWitt. Due to a hard time growing up, he'd always had to work very hard and do errands for people around their slum-like neighborhood. It was during the course of one of these errands that Andrea met one Doctor Miuth, one of the villains. Now, the villainous organization Miuth is a part of basically has a requirement for joining that you must have committed at least one hundred war crimes in the course of your turn to evil. So you have an idea what kind of people these guys are. So, "Andrea" over the years became overwhelmed with dysphoria, and due to a number of practices, as a woman, he was valued about as much as his appeal. That was when Miuth returned, and offered a "procedure" in exchange for one final "errand". In the course of this, Andrea delivered six bags to the reservoir, thus poisoning a large amount of water that was being shipped off to a "water-poor" area in another country. The poisoned water was just another experiment - he wanted to tell if he could poison water in such a way that was undetectable. In exchange for this, Miuth used a "magic plus technology" procedure on Andrea, transforming "Andrea into Andrew". Andrew hasn't seen Miuth since.
Andrew had no idea Miuth was going to essentially commit genocide, and the doctor had been utterly "affably evil" the entire time of their relationship. Andrew blames himself for being so eager to get his procedure done that he saw no evil in Miuth until it was too late. He also thinks he has "internal hatred of being transsexual" for despising the longer transition so much that he "buckled" and went to Miuth.
So, in as far as Andrew's morality, is he pretty much in the clear? Does he have any point in this much blaming of himself? Is this a "real" depiction (for whatever that is worth)?
Am I transphobic myself for thinking up this situation?

Just...general ideas about this. I figured this was the best forum for such a quandary.
^^ Andrew's dysphoria was so severe it resulted in him almost losing his life several times due to other desperate measures - he attempted suicide at least twice, when binding no longer worked as well, he started slashing his chest up "I had to make those things stop - I hated them so much". The setting, the Free City of Astray, is not hugely transphobic, but there are...other problems. Not all the considerations are entirely honest. Andrew had the option of a longer transition on pills and surgery, but he couldn't afford it / couldn't stand to wait that long. Given the kind of magic it was, only Miuth was capable of doing what he did.
The procedure itself is more nefarious than what Sleepy thinks it is. They essentially "gave Andrew new flesh" from their monstrous mooks and elite mooks, which are basically horrible troll monsters and faceless nightmare creatures, shaping it into his "new body" with no hint of the origins of this new form, but leaving the scars on his chest as a reminder that he figuratively sold his soul. Due to having what amounts to a body containing the flesh of creatures whose whole purpose is to serve the villains, they can take control of him whenever they want or even insert thoughts into his head.
For the sake of accuracy, though, Andrew had no idea Miuth was the genocidal type at the time - the villains at the time are not so well known, and all he knew of Miuth was that he was definitely a scientist. He only knew Miuth was kinda shady, but all his prior errands for the man involved bad things happening to bad people who might've in retrospect been causing inconvenience to the villains.
^ Yeah, the dysphoria is one of the most painful chapters of Andrew's life, but by no means the only painful chapter. His mother is a drunk and goes between supporting him and calling him a freak or worse. There's the dead dad thing, the fact he has to basically fight daily to put money on the table. And as stated above, the villains are capable of a lot worse than simply turning him back.
