I know what this cartoon is trying to say, but really, that is the most honest thing a doctor can say. There are a good number of medically known anomalies that can cause someone to have one set of physical sexual characteristics but have the neurological and genetic characteristics of the other.

Simple answer: Because Social Security is the name of a particular funding program, with fixed methods of obtaining Social Security funding, and "welfare" is a generalized blanket term for a wide variety of programs, each with their own methods of funding, and all varying.
