I have two.

One was in the second level of Halo: CE, I was in the tunnel and I got stuck because I didn't know that you had to jump the ramp.

The other was in the Hundred Years War mod for Medieval 2: TW. I, as England, sent up one my best armies (all units being 3 silver to 3 gold chevrons) against three rebel Welsh armies, packed with extremely good archers. I saw elite units melt in seconds, my only survivors were some cavalry that had managed to escape.

A buddy of mine in high school got the first Legacy of Cain game.  He started it up, died, and restarted.  This happened a good dozen or so times before he realized he was supposed to die for the story to progress.

I was playing Left 4 Dead 2 Versus Mode, online one night.  We, playing as human survivors, were running through an alley when one of the zombies puked on us, bluring our vision and attracting other zombies.  I tossed a pipe bomb and started melee striking anything nearby.  Then I heard a beeping behind me.  It seems that the pipe bomb had bounced off a wall and landed behind us . . . right next to fuel tanks.

It exploded, stunning us and causing a fire.  Three of us were incapacitated, right in the middle of the fire.  In my blurred vision, I saw something running toward me so I opened fire with my pistol.  Make that all 4 of us incapped.

Looking up, I saw the other team atop the building, looking down at us, and no doubt falling over laughing.  I also got a few PM's, ssaying, "Nice job, Vyper," "Way to go, Vyper!"

Surprisingly, no one brought up a vote to kick me.

Gah I just screwed up a quest in Skyrim hardcore all because I forgot to check a body. >.<

After starting Hero Mode in Skyward Sword, I managed to get killed by the Stalfos in the first dungeon about four times due to not watching my hearts closely enough. As if that weren't enough, Ghirahim killed me about three times before I managed to defeat him, and the douchebag broke my shield to boot.

Nobody mentione Soulstorm yet?  So many glitches, so little time to list them.  But having games that took an hour to load was the worst bit.
