In one New World of Darkness game, I saw a guy roll nine successes on three dice due to the 10-again rule.

What's the most frustrating thing to ever happen to you in a roleplaying game? For me, it was six months without a single successful roll.

A campaign that should have taken two hours, max, dragging on for almost a month (due to a combination of bad rolls, dumb players, and a pair of squabbling mages).
