For a short time, I had a pet piranha. Yes, I know. He came from a cousin that fled to Ohio due to some drug charges, and his father started giving away all of his possessions. One of those happened to be a large fish tank with a piranha and some other fish (a white and orange one; can't remember the type).

We named him Hannibal. After a few months of ownership, he finally ate the other fish and then died of natural causes.

Oh, that is most certainly horrible. I will gladly agree, that is a whole big pile of what-the-fuckery. Safewords are very important, people! I'd never tell my girlfriend to not use the safeword. First, that's awful. Secondly, she'd kill me, which is also awful. But, seriously, that is never acceptable.

As someone whose fetishes can make for a "lighter" version of BDSM, I gotta say, safe words are still the most important thing.  Even if your fetish is not being able to use a safe word, then have your "pretend" safe word, then a REAL safe word that you use when you actually need to.
