The things that annoys me the most is when games (specifically open world games) don't have skippable tutorials

The "Pay-to-play" model.

When a game offers a ridiculously long cool down for even basic commands, which can be sped up if you acquire the item that are rare in the game and can be purchased.

I am turned off by games that do this shit.

Games that make you physically touch items to automatically pick them up, but don't have very good detection and then place items on tables or boxes where they're slightly too far for the game to tell that you're picking them up. So you need to awkwardly hop up onto a crate to pick up something within reach.
Still can't think of a signature a year later.

Continuity errors. I don't like retons either, but at least it's (usually) acknowledged that "yeah, we said that, but we're not going to continue on that line of thought". But when a game makes errors that could have been prevented by simply glancing at what you already wrote, I'm gonna be fucking pissed.

THIS so much.  Also, having 2-3 actions assigned to the same button (such as ME3's roll/attach to wall), way more focus on multiplayer than SP even when SP is available in the game (Payday 2), enemies that lvl up with you (Dead Island, Oblivion), lack of save spots.
Whenever I hear a politician speaking strongly for or against abortion, all I hear is, "I have no idea how to fix the economy!"

(I really seriously felt like posting that video at the right moment.)

Also, in older games, doors that are open, yet you cannot enter them, so they serve no purpose.

But really seriously, when there are even doors and they are just details. We players are naturally drawn to rooms in video games and when there is nothing, there is no reason they should be there.

(I am looking at you, Half-life 2 and Portal 1 & 2. I know where Valve were going in those games, realism and realistic proportions of buildings, yada yada...)
